Job Summary:

Wisconsin Community Services, Inc. is seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will be responsible for providing intensive and individualized care coordination for children and adolescents under the Wraparound Practice Model of care. Key Responsibilities:

  • Maintain a caseload of approximately 8 children and families, providing regular visits, phone contact, and support as needed.
  • Assemble a Child and Family Team within two weeks of enrollment, identifying family members, natural supports, and service providers.
  • Develop and implement individualized Plans of Care, including comprehensive 24-hour Crisis Plans, in collaboration with the Child and Family Team.
  • Monitor the provision and quality of services, advocating for children and families across various settings, including home, educational, court, and community settings.
  • Attend court hearings as a representative of Wraparound Milwaukee, supporting children and families throughout the court process.
  • Seek community resources, modifying the Service Array (SAR) as needed, and provide or arrange for transportation for youth and families.
  • Provide or secure support and crisis/emergency services, completing necessary paperwork in a strength-based manner.
  • Maintain accurate information online, ensuring up-to-date family demographic information and Child and Family Team Member contact details.

Requirements:

  • Bachelor's degree in a relevant area of education or human services, with a minimum of one year of continued experience providing mental health services.
  • Complete 85 hours of mandatory training in Wraparound philosophy and policies, as well as attend a Family Orientation within six months of hire date.
  • Working knowledge of computer applications, including Windows and Microsoft Word.
  • Ability to display cultural competence, responding respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
  • Valid driver's license, automobile, and insurance sufficient to meet agency requirements.

Working Conditions:

This role requires working in a busy office environment, managing caseloads of children, young adults, and families. Some work is done sitting at a desk using a computer, while outreach is performed in the community and families' homes. The role involves driving and may require flexible work schedules, including evenings and weekends.
