Clinical Care Coordinator

Annual Salary Range: $45,000 - $60,000

About the Position

Main Responsibilities
  • Coordinate client transportation, schedule medical appointments, and maintain accurate records.
  • Develop and maintain strong relationships with clients, families, and external partners.
  • Collaborate with the nursing team to ensure seamless care delivery and develop effective solutions to complex problems.
  • Stay current with industry developments, best practices, and regulatory requirements to ensure compliance.
  • Participate in ongoing training and professional development to enhance skills and knowledge.
Requirements
  • Degree from a recognized Medical Assistant program or equivalent experience.
  • Excellent communication, organizational, and time management skills.
  • Ability to work independently and as part of a team.
  • Strong attention to detail and commitment to quality patient care.
  • Familiarity with electronic health records and Microsoft Office Suite.
