Key Responsibilities:- Provide comprehensive nursing care to patients in their homes, under the direction of a registered nurse.
- Participate in the development of the plan of care, collaborating with interdisciplinary team members and reporting to Field Nursing Supervisors.
- Offer health education to patients and caregivers, empowering them to take an active role in their care.
- Accept and enter verbal and written orders from physicians, obtaining co-signature from RN Case Manager as required.
- Maintain open communication with physicians, patients, family members, staff, and RNs, addressing client problems and concerns in a timely manner.
- Document care provided accurately and efficiently, ensuring seamless communication with the healthcare team.
- Active nurse license in the state of Connecticut or compact license.
- Minimum 6 months RN Pediatric experience, with RN Home Health experience preferred.
- Reliable transportation to perform job duties.
- Dedication to quality patient care.
