Position Overview

We are currently seeking a Full-Time Day Shift Nurse Navigator for Residential Services.

Schedule: 8:30 AM - 4:30 PM, with a commitment of 3 days per week at one location and 2 days per week at another.

Qualifications

Education/Training: Candidates must hold a valid, unencumbered LPN license in Pennsylvania and possess current CPR/AED certification (or be willing to obtain certification within 30 days of employment).

Skills Required: Proficiency in English, including reading, writing, and mathematics; strong interpersonal and communication abilities; and a valid driver's license.

Experience: A minimum of one year in a related healthcare position is typically required.

Key Responsibilities

The Nurse Navigator will be responsible for implementing a wellness-focused program for residents in our community. Specific duties include:

  • Conducting initial pre-admission assessments for incoming residents.
  • Meeting with residents at the time of occupancy, annually, and as needed to facilitate referrals to internal and external services that promote independence and optimal outcomes.
  • Completing necessary documentation for supplemental services as required.
  • Monitoring residents receiving supplemental services to ensure compliance with contractual obligations and safety standards, reporting any issues to the Executive Director.
  • Planning and delivering health education and wellness initiatives.
  • Coordinating on-site health service clinics for residents.
  • Leading resident review meetings to assist families in determining appropriate care levels.
  • Facilitating transitions to new care levels in collaboration with other team members.
  • Responding to inquiries from residents' families and healthcare professionals.
  • Serving as a resource for residents, ensuring effective communication with external healthcare providers.
