The Foster Care Coordinator position involves working with children and youth who are in out-of-home placement through the foster care system.

Your key responsibilities will include assessing and planning care strategies that are tailored to the individual needs of each child or youth.

You'll also be responsible for coordinating referrals for psychosocial, behavioral, and medical services, managing care plans throughout the continuum of care, and communicating with all stakeholders to ensure quality coordinated care and services are provided.

A strong advocate for children and families, you'll work to ensure their needs and choices are fully represented and supported by the healthcare team.

In this role, you'll provide trauma-informed and disease/disorder-specific education to members and caregivers, coordinate with the child welfare and juvenile justice system on behalf of members and caregivers, and coordinate with additional services such as educational/educational advocacy, complex behavioral health, and high-fidelity wraparound services.

You may also be involved in reviewing assessments and care plans for complex cases with additional oversight from a master's level case manager.

To succeed in this role, you'll need:

  • Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or related human services area
  • 3+ years of experience working with multi-system children
  • 2+ years of experience working within the community health setting in a healthcare role
  • 1+ years of experience working with MS Word, Excel, and Outlook
  • Ability to travel in assigned region to visit Medicaid members in foster homes and/or other settings
  • Reside locally to Reno, Butler, and Sedgwick Counties, KS

Salary: $60,000 - $80,000 per year
