About the Role

The Foster Care Coordinator at UnitedHealthcare plays a vital role in supporting children and youth in out-of-home placement, ensuring they receive personalized care strategies. This field-based position is located in Reno, Butler, Sedgwick Counties, KS, and surrounding areas.

Key Responsibilities

  • Assess and implement individualized care plans for children and youth in foster care, considering reunification and permanency planning needs.
  • Identify, initiate, and coordinate referrals for psychosocial, behavioral, and medical services.
  • Manage care plans and communicate with stakeholders to ensure quality coordinated care.
  • Advocate for children, youth, and families to represent their needs and choices within the healthcare team.
  • Provide trauma-informed education and coordination with child welfare and juvenile justice systems.
  • Collaborate with additional services, including educational advocacy, complex behavioral health, and state Family First Prevention and Services Act programs.

Requirements

  • Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or related human services area.
  • 3+ years of experience working with multi-system children.
  • 2+ years of experience in community health settings in a healthcare role.
  • 1+ years of experience with MS Word, Excel, and Outlook.
  • Ability to travel to visit Medicaid members in foster homes and other settings.
  • Local residence in Reno, Butler, and Sedgwick Counties, KS.

Estimated Salary: $55,000 - $65,000 per year.
