We are seeking a highly skilled Nurse Care Coordinator to join our team at The University of Kansas Health System.

About the Role

This is a full-time position that offers a competitive salary and benefits package. We estimate the salary to be around $85,000-$110,000 per year, based on experience and qualifications.

The successful candidate will have a strong nursing background and excellent communication skills. They will work closely with patients, families, and healthcare teams to coordinate care and ensure seamless transitions.

Key Responsibilities
  • Care Coordination: Develop and implement individualized care plans in collaboration with interdisciplinary teams.
  • Patient Education: Provide education and support to patients and families on their health conditions, treatment options, and self-care techniques.
  • Communication: Facilitate effective communication between healthcare providers, patients, and families to ensure comprehensive care.
  • Documentation: Maintain accurate and up-to-date documentation of patient care plans, progress notes, and other relevant information.
Requirements
  • Educational Requirements: Associate Degree in Nursing or equivalent.
  • Licensure and Certification: Current RN licensure in the state of Kansas and BLS certification.
