OVERVIEW: The Intensive Case Management Services (ICMS) are essential for supporting individuals experiencing homelessness, those at risk of losing their homes, and those with complex health or behavioral health challenges. This role is crucial for assisting high utilizers of public services and other vulnerable groups, including individuals with histories in the criminal justice system and those transitioning from institutional settings.

The ICMS team adopts a comprehensive approach to facilitate clients' journey from homelessness to stable housing. Services provided include outreach, engagement, assessment, service planning, and connections to health care, mental health support, and other essential services. Assistance with benefits, transportation, legal matters, and crisis intervention is also a key component of this role.

KEY RESPONSIBILITIES:

  • Facilitate and coordinate support services tailored to individual ICMS participants.
  • Build and maintain trusting relationships with clients, focusing on housing placement, stability, and overall well-being.
  • Assist with screening, assessment, and orientation regarding housing policies and resources.
  • Conduct comprehensive psychosocial assessments and collaborate with clients to develop personalized service plans, reviewed quarterly.
  • Utilize project management software to maintain accurate and up-to-date records and progress notes.
  • Stay informed about community resources to provide comprehensive support to clients.
  • Deliver crisis management services, including after-hours support.
  • Offer tailored referrals to medical care, mental health services, and community resources as necessary.
  • Collaborate effectively with multidisciplinary team members and participate in regular meetings and training sessions.
  • Work alongside property management to ensure compliance with lease agreements and support housing retention.
  • Assist clients in managing daily activities and appointments, fostering socialization and health maintenance.
  • Engage in training on Housing First strategies and Harm Reduction to promote successful transitions to permanent housing.
  • Contribute to program compliance and evaluation efforts with administrative staff.
  • Promote the mission and values of the organization while adhering to established policies and procedures.
  • Provide coverage for occasional evening and weekend programs as needed.
  • Perform additional duties as assigned by supervisors.

QUALIFICATIONS:

  1. Experience working with individuals facing mental health challenges, chronic health issues, and substance use disorders.
  2. Background in permanent supportive housing with diverse, low-income populations.
  3. Strong communication, organizational, and time management skills.
  4. Familiarity with eligibility requirements and application processes for federal and state benefits.
  5. Willingness to work flexible hours, including evenings and weekends.
  6. Ability to establish and report on program objectives and their effectiveness in helping clients achieve their goals.
  7. Commitment to working in a diverse community setting.
  8. Experience working with individuals and families facing mental disabilities, substance addictions, and trauma, providing services with dignity and respect.
  9. A positive attitude and self-confidence.
  10. Proficiency in word processing and database management.

EDUCATION AND EXPERIENCE:

Minimum qualifications include at least one year of experience working with homeless populations and a bachelor's degree in social work or a related field, or a minimum of two years of experience in direct mental health or intensive case management services.

COMPENSATION AND BENEFITS:

Competitive hourly wage ranging from $24.00 to $27.00. Flexible scheduling available for up to 30 hours per week. Participation in the organization's 403(b) plan and eligibility for paid time off.
