Job Description: We are seeking a highly skilled Registered Nurse to join our team as a Medical Recovery Specialist. In this role, you will be responsible for developing and implementing individualized care plans that cater to the unique needs of each patient. You will work closely with interdisciplinary teams to ensure seamless transitions and optimal outcomes for our patients. Furthermore, you will participate in discharge planning processes and communicate effectively with patients, families, and healthcare professionals.

Responsibilities:
  • Develop and implement individualized care plans for patients.
  • Collaborate with interdisciplinary teams to achieve optimal patient outcomes.
  • Participate in discharge planning processes.

Qualifications: To succeed in this position, you must have a graduate degree in Nursing from an accredited institution and a valid RN license. Prior experience in an acute care setting and ACLS certification are preferred.
