At Kaleida Health, we are seeking a highly skilled Medication History Specialist to join our team. As a Medication History Specialist, you will play a critical role in ensuring the accuracy and completeness of medication histories for patients admitted to our hospital site.

Key Responsibilities:
  • Support the patient care team in ensuring the completion and accuracy of medication histories
  • Collect and document medication information from various sources, including the patient, patient caregiver, health information exchanges, retail pharmacies, and physician offices
  • Conduct medication history documentation in various patient care settings, including emergency departments, inpatient care nursing units, and surgical pre-admission testing offices
  • Collaborate with the pharmacist, nurse, and provider to ensure the most accurate information is collected and documented
Requirements:
  • HS Diploma or GED required
  • AAS in Business/Healthcare-related degree preferred
  • BA/BS in Business/Healthcare-related degree preferred
  • 3 years of experience in a healthcare setting (within the past 5 years) preferred
  • 1 year of direct patient care in a healthcare setting preferred
  • Typing skills assessment required
Preferred Qualifications:
  • Certified Pharmacy Technician (PTCB certification)
  • Certified Medical Assistant
  • Relevant NYS Healthcare License (such as but not limited to LPN)
  • Actively enrolled in one of the following healthcare degree programs at an accredited institution: Nursing (Nurse/Nurse Practitioner), Pharmacy (Pharmacist), Medicine (Physician), Physician Assistant, Dentistry (Dentist), Dietitian/Nutritionist, Physical Therapist
Work Schedule:

Full-time position with a scheduled work hours of 11a-7p, 12-8p, 1p-9p, 2p-10p, and 3p-11p

Salary Range:

$23.76-$30.08 per hour, determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement
