Job Summary

We are seeking a compassionate and skilled Caregiver to provide direct personal care and related services to clients in their place of residence.

Responsibilities
  1. Assist clients with activities of daily living, such as bathing, hair care, skin care, and oral care.
  2. Assist clients with transfers, such as getting in-and-out of bed, and assist with ambulation.
  3. Assist clients with prescribed exercises as taught by appropriate health personnel.
  4. Assist clients with the preparation of meals, including therapeutic diets, and assist clients with eating.
  5. Assist clients with toileting, going to the bathroom, or using commodes, bedpans, or urinals.
  6. Assist clients with medications that are self-administered by the client.
  7. Perform other activities taught by a health professional for a specific patient, such as taking vital signs and non-sterile bowel and bladder hygiene care.
  8. Monitor client activities, food intake, and report changes in the client's condition and needs to the Respite Care Manager.
  9. Complete progress records and time-keeping logs regarding services performed.
  10. Perform all assigned personal care services.
  11. Perform simple household procedures essential to maintenance of proper health care at home, such as disposal of infectious secretions, cleaning of soiled linen, proper disposal of food, and light housekeeping.
  12. Accompany clients as needed to medical appointments and assist with activities of daily living.
  13. Comply with HIPAA regulations and maintain confidentiality of client and agency information at all times.
  14. Utilize client equipment and supplies safely, efficiently, and effectively.
  15. Conform to all agency policies and procedures.
  16. Maintain a comfortable work environment for all employees.
  17. Maintain proper time-keeping.
  18. Follow policies and procedures as per the Employee Handbook.
  19. Maintain all required credentials up-to-date.
  20. Report fraud and abuse.
  21. Knowledge of mandated reporting.
  22. Conduct timely recording and/or documentation of all client contact.
  23. Attend department meetings (in person, by phone, or by web).
  24. Attend all state-mandated in-service trainings.
  25. Perform other duties as assigned.
