Role Summary:
The Licensed Vocational Nurse (LVN) plays a crucial role in overseeing and coordinating medication management and resident care. The LVN also supports the Director of Nursing in establishing nursing care standards and managing resident care under physician supervision.
Work Schedule:
Part Time: Sunday and Monday 6:30 AM - 2:00 PM, Tuesday 2:30 PM - 11:00 PM
Part Time: Saturday and Sunday 2:30 PM - 11:00 PM, Monday 6:30 AM - 2:00 PM
Key Responsibilities:
- Prepare and assist residents with the self-administration of prescribed medications, ensuring adherence to individual medication care records. Organize medication carts for accuracy and efficiency during medication administration.
- Maintain medication inventory, monitor expiration dates, and ensure proper storage. Adhere to controlled substance protocols and ensure the appropriate disposal of expired or excess medications.
- Evaluate medication administration and inform the Director of Nursing of any potential issues with dispensing instructions or adverse drug interactions. Ensure that all resident medication records are accurate and complete.
- Assist in resident examinations and treatments, executing orders as directed by the Director of Nursing or their designee. Report any changes in residents' physical or mental conditions to the Director of Nursing and relevant neighborhood directors.
- Support the Director of Nursing with scheduling and follow-up on physician appointments, ensuring accurate recording and execution of medication orders.
- Maintain knowledge of emergency procedures and respond to emergencies as needed. Provide first aid and/or CPR when required.
- Ensure accuracy in all resident records.
- Complete and sign necessary documentation, including resident care notes, medication administration records, and incident reports, providing follow-up as necessary.
- Uphold confidentiality regarding resident and community information.
- Conduct interviews and assessments of current residents to evaluate needs, monitor and document medical conditions and behavioral changes, and recommend adjustments to care plans.
- Participate in the planning of resident move-ins, transfers, and discharges.
- Collaborate effectively with residents, their families, community staff, physicians, consultants, and ancillary service providers. Act as a liaison between the community and residents' supervising physicians.
- Guide neighborhood team members and residents in executing emergency and disaster procedures during drills and actual events.
- Provide daily updates to the Director of Nursing regarding community conditions, resident statuses, and nursing supply needs.
- Engage in professional development activities as required.
- Report any suspicions or evidence of elder abuse, mistreatment, or neglect as outlined in company policies.
Qualifications and Skills:
- Excellent verbal communication skills and the ability to work effectively in a team environment.
- Strong organizational and time management skills.
- Ability to perform calculations related to budgets, staffing, and medication dosages.
- Knowledge of relevant healthcare regulations at the federal and state levels.
- Proficient in computer applications, including Word, Excel, Outlook, PowerPoint, and Property Management Systems.
Requirements:
- Ability to communicate clearly and positively with individuals of all ages.
- Proficient in reading and writing English, with the ability to follow verbal and written instructions.
- A minimum of three years of experience in a similar role, preferably in retirement living or home care.
- Willingness to work flexible hours, including weekends and evenings.
- Ability to read and interpret professional journals, technical procedures, and regulations.
- Capability to write reports, business correspondence, and procedure manuals.
- Ability to effectively present information and respond to inquiries from various stakeholders.
- High school diploma required; college degree preferred.
- Current LVN licensure.
- Good physical and mental health, verified by a recent health screening.
- Freedom from pulmonary tuberculosis, confirmed by a recent health screening.
- Criminal background clearance through DOJ and FBI via Live-Scan prior to employment.
