Position Summary:
The Licensed Vocational Nurse (LVN) is accountable for overseeing and coordinating medication management and resident care services. The LVN also supports the Director of Nursing in establishing nursing care standards and managing resident care under a physician's guidance.
Work Schedule:
Part-Time: Sunday and Monday 6:30 AM - 2:00 PM, Tuesday 2:30 PM - 11:00 PM
Part-Time: Saturday and Sunday 2:30 PM - 11:00 PM, Monday 6:30 AM - 2:00 PM
Key Responsibilities:
- Facilitates and accurately assists residents with self-administered medications in line with individual medication care records. Organizes medication carts to enhance accuracy and efficiency during medication distribution.
- Maintains an inventory of necessary medications, monitors expiration dates, and ensures proper storage. Adheres to controlled substance protocols and ensures the correct disposal of expired or contaminated medications.
- Evaluates medication administration and alerts the Director of Nursing to any potential issues with dispensing instructions or adverse drug interactions. Ensures that all resident medication records are complete and accurate.
- Assists in resident examinations and treatments, executing orders as directed by the Director of Nursing or their designee. Communicates any changes in residents' physical or mental conditions to the Director of Nursing and relevant neighborhood directors.
- Supports the Director of Nursing with scheduling and follow-up on physician appointments, ensuring that medication orders are accurately documented and executed.
- Maintains a thorough understanding of emergency protocols and responds to all emergency situations. Performs first aid and CPR as necessary.
- Ensures the accuracy of all resident records.
- Completes and signs necessary documentation, including resident care notes, medication administration records, and incident reports, providing follow-up as needed.
- Upholds the confidentiality of resident and community information.
- Conducts interviews and assessments of current residents to evaluate needs, monitors and documents changes in medical conditions and behaviors, and participates in the formal reassessment of resident care needs, recommending adjustments to care plans.
- Engages in the move-in, transfer, and discharge planning processes for residents.
- Collaborates effectively with residents, their families, community staff, physicians, consultants, and ancillary service providers, acting as a liaison with the community and residents' supervising physicians.
- Guides neighborhood team members and residents in executing emergency and disaster procedures during drills and actual events.
- Provides daily updates to the Director of Nursing regarding community conditions, resident statuses, and nursing supply needs.
- Participates in required professional development activities.
- Reports any suspicions or evidence of elder abuse, mistreatment, or neglect as outlined in personnel policies.
Required Knowledge and Skills:
- Exceptional verbal communication skills.
- Ability to work effectively in a team-oriented environment.
- Strong organizational and time management skills.
- Proficiency in calculating figures related to budgets, staffing, and medication dosages.
- Knowledge of relevant health care and employment regulations at both Federal and State levels.
- Proficient in computer applications, including Word, Excel, Outlook, PowerPoint, and Property Management Systems.
Qualifications:
- Ability to communicate clearly and effectively with individuals of all ages while maintaining a positive and energetic demeanor.
- Proficient in reading and writing English, with the ability to follow verbal and written instructions.
- A minimum of three (3) years of experience in a similar role, preferably in retirement living or home care.
- Willingness to work flexible hours, including weekends and evenings.
- Ability to read, analyze, and interpret human service periodicals, professional journals, technical procedures, and governmental regulations.
- Capability to write reports, business correspondence, and procedural manuals.
- Ability to effectively present information and respond to inquiries from managers, clients, customers, and the general public.
- A high school diploma is required; a college degree is preferred.
- Must hold a valid LVN license.
- Must be in good health, physically and mentally capable of performing assigned tasks, with health verified by a physician's screening.
- Must demonstrate freedom from pulmonary tuberculosis through a health screening conducted by a physician.
- Must pass a criminal background check via DOJ and FBI prior to initial employment.
