Job Opportunity: Registered Nurse (RN) or Licensed Vocational Nurse (LVN) - Pediatric Home Health

Thrive Skilled Pediatric Care LLC is seeking a compassionate and skilled Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to provide exceptional care to our pediatric patients in the Sour Lake, TX area.

Key Responsibilities:
  • Deliver high-quality clinical home care to medically fragile children, developing and implementing individualized care plans to meet their unique needs.
  • Collaborate with healthcare teams to ensure seamless care coordination, conducting thorough assessments and evaluations to identify patient needs and develop effective care plans.
  • Document patient progress and communicate with families and healthcare teams, providing education and support to ensure optimal patient outcomes.
