Responsibilities- Provide one-on-one care to pediatric patients in their homes, promoting their physical, emotional, and developmental well-being.
- Develop and implement individualized care plans, working collaboratively with patients, families, and healthcare teams.
- Administer medications, perform medical procedures, and provide wound care as needed.
- Monitor patients' conditions, reporting any changes or concerns to healthcare providers.
- Collaborate with Thrive's interdisciplinary team to ensure seamless care coordination and communication.
- RN/LVN with a current, valid license to practice nursing in the state of Texas.
- Current CPR certification.
- Experience working with pediatric patients, preferably in a home health setting.
