As a Pediatric Home Health Nurse, you will be responsible for delivering personalized care to our patients, working collaboratively with our interdisciplinary team to ensure seamless care coordination, and upholding our company's core values of Excellence, Respect, Integrity, Compassion, and Social Responsibility.
Responsibilities:
- Provide one-on-one care to pediatric patients in their homes, focusing on their unique needs and goals.
- Develop and implement individualized care plans, working closely with our patients' families and healthcare teams.
- Administer medications, perform medical procedures, and provide wound care as needed.
- Monitor patients' conditions, reporting any changes or concerns to our clinical team.
- Collaborate with our interdisciplinary team to ensure comprehensive care coordination, including therapy services, medical supplies, and equipment.
- Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements:
- RN/LVN with a current valid license to practice as issued by the State Board of Nursing.
- Current CPR certification.
