Job Summary

We are seeking a highly skilled Transfer Center Specialist to join our team at Halifax Health. As a key member of our healthcare team, you will be responsible for coordinating transfers in accordance with state and federal guidelines, processing physician consultation requests, and providing exceptional customer service.

Key Responsibilities
  • Coordinate transfers in accordance with state and federal guidelines
  • Process physician consultation requests
  • Provide outstanding customer service
  • Effectively communicate with physicians, peers, and nursing leadership
  • Handle physician consults
  • Utilize computer skills to input data, retrieve data, and prepare reports
  • Evaluate, coordinate, monitor, and document all activities associated with the transfer process
  • Coordinate communication between the referring physician and the accepting physician
  • Maintain working knowledge of current Trauma criteria and EMTALA guidelines
  • Notify House Nursing Supervisor immediately of transfer-related concerns and document in the occurrence reporting system
  • Appropriately prioritize placement of internal transfers, direct admissions, ED, and PACU patients
  • Provide patient data to physicians and Care Coordinator as requested
  • Coordinate activities to notify appropriate physician of consult within targeted timeframe
  • Perform accurate entry of physician consults in a timely manner
  • Utilize Vocera, hands-free headsets, and call recording system per department guidelines/policies
Requirements
  • High School Diploma or GED equivalent required; Associate's degree in healthcare-related field preferred
  • One year of experience in a hospital setting preferred
  • Business office experience preferred
  • Strong verbal/written communication skills and excellent customer service required
  • Ability to handle high levels of stress due to varying and unpredictable situations
  • Strong knowledge of medical terminology
  • General computer and Microsoft Office knowledge and skills required
