Residential Care Coordinator

We are seeking a highly skilled and compassionate Residential Counselor/Medical Coordinator to join our team at Constructive Partnerships Unlimited. This dynamic individual will provide exceptional care and support to individuals with developmental disabilities, while also assisting with medical-related tasks and responsibilities.
ResponsibilitiesA. Residential Counselor Responsibilities
- Assist individuals with activities of daily living, including toileting, changing adult diapers, lifting/transferring, showering, grooming, oral hygiene, meal programs, and dressing.
- Participate in in-house recreational activities and accompany individuals to community-based activities.
- Participate as a member of the interdisciplinary team and assist in evaluating and implementing goals/outcomes.
- Maintain records, charts, and logs for each individual, including changes in behavior, goal progress/data collection, community inclusion, fire drills, vital signs, blood pressure readings, blood sugar readings, bowel regimens, etc.
- Prepare and serve meals for individuals, following individualized menus and cooking instructions.
- Maintain cleanliness and safety of the residence and individuals' possessions, performing general housekeeping tasks.
- Administer medications and/or treatments, and record same.
- Ensure the well-being and care of individuals, responding to changes in conditions and participating in fire drills.
- Accompany individuals to medical appointments, outside programming, recreation programs, inclusionary activities, etc.
- Attend all required in-service trainings and staff meetings.
- Report all incidents to the Residence Manager and report all medical issues/injuries to the registered nurse.
B. Medical Assistant Responsibilities
- Maintain medical appointment flow sheets for all assigned individuals, indicating all medical appointments.
- Routinely review Clinic Visit Summaries (CVS's) and ensure that prescriptions for all prescribed medications are in the individual's record and up-to-date.
- Report any concerns/discrepancies to the nurse case manager and ensure all over-the-counter medications are not expired.
- Schedule medical appointments, escort individuals, and arrange transportation as necessary.
- Maintain medical records, including obtaining/filing all necessary reports and maintaining a database of consumer health information.
- Complete clinic visit summaries when verbally directed by the nurse.
- Meet regularly with the nurse regarding medical follow-up issues and personnel issues.
- Ensure that CVS's are present at the time of appointment and contain all necessary medical documentation.
A. Qualifications
- High school diploma or GED and prior experience working with individuals with developmental disabilities.
- Proficient in Microsoft Word and capable of learning new software applications.
- Excellent organizational skills.
- Ability to understand and communicate in English.
- Excellent oral and written communications skills.
- Ability to travel to and access multiple locations throughout the metropolitan area.
- Ability to work flexible hours and/or days.
- Demonstrate satisfactory completion of a specialized course pertinent to the individual's needs, as necessary, and maintain annual re-certification.
- NYS Valid Driver's License & Proof of Department of Motor Vehicles driving history, 3 years of current driving experience with no more than one moving violation in the previous 24 months, and no driving while intoxicated convictions required.
B. Essential Physical Requirements
- The ability to coordinate the movement of the arms, legs, and torso together in activities where the whole body is in motion.
- The ability to lift individuals to/from various surfaces safely.
- The ability to perform a two-person transfer safely.
- The ability to lift and reach common objects such as laundry and nonperishable food items.
- The ability to push/pull, turn, and maneuver equipment, including occupied wheelchairs.
- Possess strength, coordination, and agility to safely and quickly respond to emergency situations.
- The ability to bend, stretch, rotate, or reach out with the body, arms, and/or legs to ensure the safety of individuals in all situations.
- Ability to operate up to a 15-person van, based on program need required.
C. Operate Machine/Equipment
- Office Equipment (includes Fax, Copier, and Scanner, tablets, and Personal Computer)
- Household Appliances (Washing Machine/Dryer, Kitchen Appliances, etc.)
- Mechanical Lifter (Hoyer Lift, Wheelchair Lift, etc.)
- Various adaptive equipment (wheelchairs, walkers, bathroom equipment, splints, etc.)
