Residential Services Coordinator 3 Position

About the Role:
The DePaul Group is seeking a dedicated and compassionate Mental Health Professional to fill the position of Residential Services Coordinator 3. This role is a vital part of our team, providing direct support, advocacy, and education to residents in our program.
Key Responsibilities:
- Work with management staff to provide tours and lunch visits to prospective residents, and provide input to the Admission/Management team regarding individual acceptance into the program.
- Ensure each resident receives comprehensive service planning in a person-centered approach, including delivery of recovery-oriented services and ongoing discharge planning practices.
- Conduct quarterly service plan reviews with residents and their support teams to assess progress and make revisions as needed.
- Provide teaching, monitoring, and recovery-oriented supports to residents, including rehabilitative services such as assertiveness/self-advocacy training, community integration/resource development, and daily living skills training.
- Observe, monitor, document, and report resident behaviors and progress in accordance with agency standards.
- Maintain accurate and timely resident files, including documentation of progress notes, service plans, and other pertinent information.
- Ensure annual appointments for physicals, eye exams, medical specialist appointments, and dental appointments are scheduled and transportation is arranged as needed.
- Work collaboratively with residents' Health Home Care Managers and other community service providers to ensure continuity of care.
- Serve as a key liaison between residents and their clinical providers regarding monitoring of mental health and well-being.
- Provide direct assistance to residents with daily living skill training, including upkeep of resident rooms, meal preparation, and laundry assistance.
- Help residents plan for purchasing clothes and other necessities.
- Work with residents to encourage healthy connections with family and communities, and develop new supports and interests.
- Coordinate and participate in residents' recreational activities as needed.
- Assist Medication Coordinators with supervision of resident medications and assume dining room responsibilities during breakfast as assigned.
- Provide safe transportation of residents as needed.
- Respect and maintain resident confidentiality, and demonstrate a caring and positive attitude toward all residents, staff, guests, family members, and service providers.
- Follow all safety rules and regulations for self, residents, and staff.
- Attend staff meetings and trainings as required by supervisor.
- Work as a member of the residential team for each resident, providing relevant input for treatment/support team meetings and continued stay reviews.
Requirements:
This position requires a Bachelor's degree in a Human Services field, plus at least one year of experience working in a Human Services field. A clean/valid New York State driving record is also required.
Work Environment:
The schedule for this position is Monday-Friday, 7am-4:30pm. This position is eligible for various benefits, including medical insurance, dental insurance, vision insurance, life insurance, generous PTO, paid holidays, 403B employer match, and employee discount programs.
