Overview

DePaul Community Residence-Single Room Occupancy (CR-SRO) Program is seeking a dedicated mental health professional to provide direct support, advocacy, and education to residents in the evening shift. The Residential Services Coordinator will assist residents with developing individualized service plans and coordinate services to enhance their autonomy, quality of life, and recovery process.

Responsibilities

  • Work with management staff to provide tours and lunch visits to prospective residents and provide input on their acceptance into the program.
  • Ensure comprehensive service planning in a person-centered approach, including delivery of recovery-oriented services and ongoing discharge planning practices.
  • Conduct quarterly service plan reviews with residents and their support teams to assess progress and make revisions as needed.
  • Provide teaching, monitoring, and recovery-oriented supports to residents, including rehabilitative services such as assertiveness training, community integration, and daily living skills training.
  • Observe, monitor, document, and report resident behaviors and progress in accordance with agency standards.
  • Maintain accurate and timely resident files, including progress notes, service plans, and psychiatric and medical records.
  • Ensure annual appointments for physicals, eye exams, medical specialist appointments, and dental appointments are scheduled and transportation is arranged as needed.
  • Work collaboratively with residents' health home care managers and other community service providers to ensure continuity of care.
  • Serve as a key liaison between residents and their clinical providers to monitor their mental health and well-being.
  • Provide direct assistance with daily living skills training, including upkeep of resident rooms, meal preparation, and laundry assistance.
  • Help residents plan for purchasing clothes and other necessities.
  • Encourage healthy connections with family and communities and develop new supports and interests.
  • Coordinate and participate in recreational activities as needed.
  • Assist medication coordinators with supervision of resident medications and assume dining room responsibilities during breakfast as assigned.
  • Provide safe transportation of residents as needed.
  • Respect and maintain resident confidentiality and demonstrate a caring, positive attitude toward all residents, staff, guests, family members, and service providers.
  • Follow all safety rules and regulations for self, residents, and staff.
  • Attend staff meetings and trainings as required by supervisor.

Qualifications

Education and Experience: Meet one of the following criteria:

A. Bachelor's degree in a Human Services field plus at least one year of experience working in a Human Services field.

B. Associate's degree in a Human Services field plus at least three years of related experience working in a Human Services field.

C. High School diploma/GED plus at least five years of experience working in a Human Services field.

Must have a clean/valid New York State driving record and be at least 18 years old.

Work Environment

Two shifts available: Tuesdays-Thursdays and Saturdays (12:30-11pm) and Wednesdays-Saturdays (12:30-11pm).
