Job Summary

We are seeking a highly skilled and compassionate Social Services Specialist to join our team at Marshwood Center in Lewiston, ME. As a key member of our interdisciplinary team, you will work closely with patients, residents, and their families to identify and address their social, emotional, and mental health needs.

Key Responsibilities:
  • Conduct comprehensive psychosocial assessments to identify patients' strengths and needs
  • Develop and implement individualized care plans to promote patients' well-being and quality of life
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
  • Provide education and support to patients, residents, and their families on social services and psychosocial needs
  • Stay up-to-date with federal, state, and local guidelines and regulations, policies, and procedures
Requirements:
  • Bachelor's degree in social work or human services field required
  • Must possess any certifications/licensures as required by State of employment to practice in long-term care
