Job Summary

We are seeking a highly skilled and compassionate Social Services Specialist to join our team at Marshwood Center in Lewiston, ME. As a key member of our interdisciplinary team, you will work closely with patients, residents, and their families to identify and address their social, emotional, and mental health needs.

Key Responsibilities:
  • Conduct comprehensive psychosocial assessments to identify patients' strengths and needs
  • Develop and implement individualized care plans to promote patients' well-being and quality of life
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
  • Provide education and support to patients, residents, and their families on various topics, including aging and disability
  • Stay up-to-date with relevant laws, regulations, and industry standards to ensure compliance and best practices
Requirements:
  • Bachelor's degree in social work or a related field
  • State-specific certifications or licenses as required
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks
