Key Responsibilities- Conduct thorough psychosocial evaluations to identify patient needs and develop individualized treatment plans.
- Provide individual, family, and group therapy sessions to support patient recovery and well-being.
- Collaborate with the treatment team to ensure seamless care coordination and effective communication.
- Document patient progress and treatment plans in a timely and accurate manner.
- Participate in quality improvement initiatives to enhance the overall quality of care.
- Masters degree in Social Work or related field.
- Licensure preferred, but not required (must obtain within 6 months of start date).
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
