We are seeking a highly skilled and compassionate Senior RN Patient Care Manager to join our team at Heartland Hospice. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our hospice site, ensuring the provision of high-quality care to our patients and their families.
Key Responsibilities- Administer the clinical aspects of the hospice program and ensure the provision of quality care to patients.
- Manage, oversee, and direct the services and performance of assigned team members.
- Supervise, manage performance, and direct the work of specified team members, assigning admit and evaluation visits, scheduling visits, and ensuring staff are completing visits in a timely manner.
- Manage the site in the absence of the Executive Director or Administrator.
- Manage and oversee clinical and administrative functions, including recordkeeping.
- Respond to phone referrals and inquiries in a timely, professional, and compassionate manner.
- Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.
To be successful in this role, you will need:
- A degree in nursing.
- Registered Nurse with ASN or BSN from an accredited school of nursing.
- One year of experience as an RN (Licensed in the state of requested employment).
- Three years of experience in direct patient care.
- One year of management experience.
- Two or more years of experience in hospice or home care.
