RN Care Manager

We are seeking a skilled RN Care Manager to join our team at NorthShore University HealthSystem. The successful candidate will be responsible for facilitating payor authorization processes, ensuring accurate patient type assignment, and collaborating with the CDI team to ensure accurate documentation.
Key Responsibilities:
Facilitate payor authorization processes and ensure accurate patient type assignment
Collaborate with the CDI team to ensure accurate documentation
Work with physicians, nursing, and other members of the multi-disciplinary care team to effect timely, appropriate patient progression
Address and resolve system problems impeding diagnostic or treatment progress
Proactively identify and resolve delays and obstacles to discharge to optimize length of stay
Monitor patient progress and intervene as necessary to ensure high-quality, efficient, and cost-effective care
Requirements:
Current professional Nursing Licensure in the state of IL required
Bachelor's degree in Nursing from a NLN accredited school of nursing preferred
Clinical certification, such as case management certification, beneficial
Basic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross) certification required
Minimum of 2 years clinical nursing experience required; previous experience in Utilization Management, Managed Care, or chronic disease management highly preferred
