Nurse II Job Description:

To provide safe and effective resident care, uphold resident rights, and comply with state regulations and recognized nursing care standards, while collaborating with physicians, therapists, and healthcare providers to develop and implement individualized care plans.

Key Responsibilities:

  • Lead daily clinical and administrative functions, including resident care needs assessment, quality standards, and resident and family satisfaction evaluation.
  • Develop and implement training on human behavior, performance, and psychological research methods, as well as the assessment and treatment of behavioral and affective disorders.
  • Collaborate with the Leadership Team to identify complex problems, review related information, and develop and evaluate solutions.
  • Provide supervision, guidance, and support to care team members, including hiring, training, evaluating, and progressive discipline.
  • Ensure effective communication with residents, families, and team members, and maintain accurate documentation of resident care and health concerns.
  • Perform skilled nursing tasks, provide support and assistance with medication management, and ensure safe and effective resident care.
  • Review and develop care plans, monitor resident health concerns, and ensure timely and accurate input of staff work schedules.

Requirements:

  • Licensed Nurse in good standing with State Licensing Authority.
  • Experience in training and team member development preferred.
  • Minimum of 2 years' experience working in dementia care or the coexistence of two or more chronic diseases in an individual.
  • Ability to establish effective relationships with residents, family members, and team members.
  • Strong documentation skills, basic computer skills, and ability to communicate effectively with residents, families, and team members.
