Job Summary

Ochsner Health System is seeking a skilled LPN Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality patient care and experience.

Key Responsibilities
  • Oversee the disease registry database to ensure accurate and up-to-date patient information.
  • Conduct pre-visit chart reviews to identify patient needs and develop personalized care plans.
  • Coordinate care across the continuum, collaborating with healthcare providers and teams to ensure seamless transitions and optimal patient outcomes.
  • Participate in quality improvement activities to drive process improvements and enhance patient satisfaction.
  • Perform other related duties as required to support the delivery of high-quality patient care.
Requirements
  • Graduate of an accredited school of practical nursing.
  • 2 years of ambulatory care experience, with a preference for 3 years of experience in ambulatory care and home health care.
  • Current licensed practical nurse (LPN) license in the state of practice.
  • Proficiency in using computers, software, and web-based applications.
  • Effective verbal and written communication skills, with the ability to present information clearly and professionally to varying levels of individuals.
  • Ability to be highly motivated, eager to learn new skills, and willing to obtain certification.
  • Organizational, decision-making, and delegating skills.
  • High level of positive interpersonal and public relation skills.
  • Leadership skills and ability to set priorities.
Physical and Environmental Demands

The physical essential functions of this job include frequently exerting 10 to 20 pounds of force to move objects, and occasionally exerting up to 100 pounds of force. Physical demand requirements are in excess of those for sedentary work. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Duties performed routinely require exposure to blood, body fluid, and tissue. The incumbent works in a patient care area, works in an area where patients enter, works directly with patients, and/or works with specimens that could contain communicable diseases. There may be an occupational risk for exposure to communicable diseases.

Because the incumbent works within a healthcare setting, there may be an occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning, and/or disposal of contaminated waste. The risk level of exposure may increase depending on the essential job duties of the role.
