We are seeking a skilled Nurse Case Manager to join our team at St. Mary's General Hospital. As a key member of our healthcare team, you will be responsible for ensuring the quality and resource management of all patients admitted to our facility.
Responsibilities- Promote cost containment and demonstrate leadership to integrate healthcare providers for seamless delivery of care.
- Perform holistic and comprehensive admission and concurrent review of medical records for medical necessity, intensity of service, and severity of illness.
- Collaborate with healthcare providers to achieve quality, clinical, and cost-effective outcomes.
To be successful in this role, you will need:
- Minimum 5 years of work experience post-graduation from an accredited school of nursing and a current state Registered Nurse license.
- Experience in case management, discharge planning, or nursing management is preferred.
- Knowledge of Milliman Criteria and InterQual Criteria is a plus.
- Basic to intermediate computer skills and ability to maintain current certifications.
