Job Summary

As a Senior Case Manager at Prime Healthcare, you will play a vital role in providing community-based mental health case management services. Your primary responsibility will be to provide direct care services, including individual and group counseling, support, and other activities. You will also participate in comprehensive assessments, develop treatment plans, and maintain clinical documentation. Additionally, you will participate in and facilitate family and collateral meetings, team meetings, and case conferences to ensure continuity of care.

Responsibilities

Provide community-based mental health case management services
Conduct comprehensive assessments and develop treatment plans
Provide direct care services, including individual and group counseling, support, and other activities
Maintain clinical documentation and participate in case conferences
Collaborate with multidisciplinary teams to ensure continuity of care

Qualifications

Master's degree in psychology, social work, or rehabilitation
Two years of mental health experience
Valid New Jersey Driver's License
