Job Summary

Eva's Village seeks a skilled Case Manager to join our team. The successful candidate will be responsible for building a consistent caseload of 30 individuals, providing outreach and engagement services, and securing government assistance benefits, social services, and housing for clients.

Key Responsibilities

  1. Embodies Eva's Village's core values and provides exceptional service to clients, funders, employees, and volunteers.
  2. Performs procedures accurately and efficiently for clients in accordance with The CAN guidelines.
  3. Provides outreach and engagement services to assist clients in accessing prevention, treatment, and recovery support services.
  4. Secures and maintains government assistance benefits, social services, unemployment, and social security cases when applicable.
  5. Assists clients in obtaining birth certificates, social security cards, and judgment of convictions.
  6. Secures and maintains client charity care when needed.
  7. Secures temporary sheltering, transitional, supportive, and permanent housing for clients.
  8. Documents progress and services provided for clients through HMIS.
  9. Maintains up-to-date client files.
  10. Participates in program data collection and maintains program tracking mechanisms.
  11. Verifies insurance benefits for clients for specific treatment and care options.
  12. Coordinates claim submissions, follow-up calls, and appeals for denied claims.
  13. Communicates with Parole, Probation, Drug Court, and other agencies as necessary.
  14. Contacts clients and referring agencies when clients miss appointments and reschedules as needed.
  15. Makes appropriate referrals based on client needs.
  16. Assists clients with legal issues, scheduling fine payments, rescinding warrants, and rescheduling court dates.
  17. Works within the community and serves as a liaison with outreach teams and other agencies.
  18. Partners with local organizations to increase housing placements and stability among referred individuals.

Qualifications

Education: Bachelor's degree in a human services field with 1 year of experience in case management.

Experience: Knowledge of the biopsychosocial disease model of substance abuse/dependence and mental health disorders. Knowledge of prevention, treatment, and recovery support services resources available in Paterson and Passaic County. Understanding of confidentiality rules and regulations related to substance abuse/dependence and mental health disorders prevention and treatment. Ability to speak, read, and write English. Must be able to work independently and complete assignments with minimal supervision. Good interpersonal skills required. Must maintain a professional demeanor and perform duties effectively in stressful/emergency situations. Must be able to maintain objectivity and deal diplomatically with clients and family members.
