We are seeking a compassionate and energetic MDS Coordinator to join our team at Big Horn Rehabilitation and Care Center. As a key member of our management team, you will engage with residents, learn their stories, and ensure they receive the best care in the industry.
As the MDS Coordinator, you will be responsible for:
- Coordinating the Resident Assessment Instrument (RAI) process
- Working closely with the healthcare team to ensure accurate and timely assessments
- Developing and implementing care plans that meet the unique needs of each resident
Requirements:
- Registered Nurse with healthcare experience in a long-term care facility
- Knowledge of RAI process required
- Knowledge of Medicare preferred
Physical Demands and Environment:
The employee is frequently required to stand, walk, use hands or fingers, reach with hands and arms, feel, talk, and hear. The employee will occasionally assist to lift and/or transfer residents weighing 100+ pounds. The employee may occasionally be exposed to blood or other body fluids. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
