JOIN THE WILLOWS AT HOWELL:
The Senior Living Services Coordinator oversees the health and wellness of each resident in our assisted living community. This role includes conducting resident evaluations, managing medication and pharmacy needs, and assisting with daily living activities and personal care. The Coordinator is responsible for hiring and supervising nursing personnel within the assisted living unit, ensuring that all services are delivered with the utmost consideration for the health, safety, and satisfaction of our residents.

Key Responsibilities:
  • Collaborates with the Director of Health Services to manage the clinical operations across the campus.
  • Ensures completion of pre-admission assessments for residents prior to their move-in and monitors ongoing health changes.
  • Leads the assessment of lifestyle and care needs to identify the medical and personal care requirements of residents.
  • Assists residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and dining.
  • Maintains timely updates to CareTracker information and ensures documentation is completed promptly.
  • Oversees documentation processes and addresses any changes in resident needs.
  • Facilitates resident meetings and addresses feedback and concerns in a timely manner.
  • Coordinates pharmacy and medication needs, ensuring compliance with state regulations and company policies.
  • Acts as a liaison to communicate effectively between residents, health campus staff, external healthcare providers, and family members.
  • Supervises and participates in the execution of treatment plans as directed by physicians.
  • Ensures that resident medical records are maintained in accordance with state guidelines.
  • Assists in training nursing assistants in providing daily living assistance and adhering to health-related policies and procedures.
  • Supports the recruitment and selection process for nursing staff in the assisted living unit.
  • Collaborates with health services leadership in hiring and training clinical staff.
  • Ensures compliance with CPR/First Aid training requirements as per state regulations.
  • Meets all orientation and training requirements as mandated by state regulations and company policies.
  • Provides weekend management coverage and on-call duties as needed.
  • Assists in the development and execution of marketing strategies for the campus.
  • Builds relationships with key referral sources and distributes campus information as necessary.
  • Serves as a point of contact for inquiries and tours related to resident admissions.
  • Participates in various health campus committees as required.
  • Engages in annual in-service training programs as mandated by the company.
  • Minimal travel may be required.
Qualifications:
  • LPN with a degree from an accredited nursing program.
Licenses/Certifications:
  • Current unencumbered LPN license in the state of practice.
  • Valid CPR certification.
  • Valid driver's license with the ability to operate a large van/bus.
