Job Summary

Trinity Health is seeking a highly skilled and dedicated Behavioral Health Telehealth Program Coordinator to join our team. As a key member of our team, you will be responsible for providing clinical and operational oversight of our Consult Liaison telehealth services grant. This role requires a strong understanding of behavioral health principles, excellent communication skills, and the ability to work effectively with patients, families, and healthcare teams.

Key Responsibilities
  • Develop and implement clinical workflows, policies, and procedures to optimize telehealth services.
  • Provide training and education to colleagues on high-quality service delivery.
  • Collaborate with internal and external teams to ensure seamless integration of services.
  • Collect and report data in compliance with grant requirements.
  • Complete behavioral health assessments and develop care plans in conjunction with telehealth psychiatrists.
  • Provide short-term psychotherapy and counseling as needed.
  • Support a recovery-focused environment and patient-centered treatment approach.
  • Work with Community Health Navigators to ensure steady referral stream and coordination of services.
  • Ensure accurate and timely documentation of clinical information in the EHR.
Requirements
  • Master's degree in counseling, psychology, or social work.
  • 3+ years of experience in behavioral health/collaborative care, with grant experience preferred.
  • Licensed/certified in the state of Michigan and adherence to the code of ethics for area of licensure.
