OVERVIEW

Under the guidance of the Director/Manager, this role is responsible for managing the daily functions of the department, including supervising staff. As a vital member of the interdisciplinary team, the individual will apply social work expertise to support the psychosocial health and welfare of residents and their families. This position also involves making recommendations to the Senior Director regarding policy and procedure enhancements and staffing needs for effective Social Work Services. The role emphasizes delivering exceptional customer service in alignment with the organization's core values.

SCHEDULE: Monday-Friday, Daytime Hours

QUALIFICATIONS
  • Bachelor's Degree in Social Work; Master's Degree preferred.
  • A minimum of 3 years of experience in a healthcare environment, particularly with elderly populations, is preferred.
KEY RESPONSIBILITIES
  • Leadership. Manages daily departmental operations, providing guidance, coaching, and support to staff in resolving issues and addressing customer concerns. Participates in the recruitment, hiring, and onboarding of new staff, ensuring their proficiency in delivering Social Work Services and engaging in performance evaluations.
  • Assessment. Conducts pre-admission and admission processes that encompass education, marketing, and immediate assistance for residents and families transitioning into the facility. Performs comprehensive psychosocial evaluations to identify needs and develop appropriate interventions and services.
  • Collaboration. Works collaboratively with the interdisciplinary team to create care plans that address the psychosocial needs of residents and their families, promoting self-determination and informed decision-making. Engages the entire team in understanding and meeting these needs, ensuring resident preferences are prioritized.
  • Documentation. Completes all necessary clinical and non-clinical documentation in compliance with professional, regulatory, and organizational standards. Contributes to departmental and organizational planning and policy formulation, collaborating with team members to identify psychosocial, cultural, and environmental factors critical to delivering high-quality care.

PHYSICAL DEMANDS:
This position is classified as Sedentary Work, requiring the ability to exert up to 10 pounds of force occasionally. The role primarily involves sitting, with occasional walking or standing.

COMPENSATION:
Salary range is $57,000 to $79,500.
