United Counseling Service is seeking a skilled Case Manager CRT to provide comprehensive support services to chronically mentally ill adults living in the community.
Key Responsibilities:- Client Support and Service Coordination:
- Develop and implement individualized service plans to meet the unique needs of assigned clients.
- Provide case management, service coordination, and supportive counseling to facilitate clients' daily living skills, housing needs, and access to income support and entitlements.
- Facilitate peer supports, community and family support, and mental and physical health services.
- Client Identification and Outreach:
- Identify and engage with community members in need of support services.
- Develop and implement outreach strategies to provide services to area clients.
- Advocate for clients' rights and provide protection whenever possible.
- Provide clients with referral information and assist with transportation as needed.
- Information Management and Record Keeping:
- Complete clinical and administrative forms for billing, information, and clinical records.
- Ensure timely completion of case management documentation and maintain confidentiality on all cases.
- Communication and Collaboration:
- Ensure effective communication with all stakeholders involved in clients' care and planning.
- Education and Experience:
- Preferred Bachelor's degree in Social Work, Psychology, or related Human Services field.
- Equivalent experience of 1-3 years will be considered.
- Licenses and Certifications:
- Valid Driver's License.
- Computer Skills:
- Proficiency in typing and navigating computer systems.
