Job SummaryThe Recovery Center of Kentucky is seeking a highly skilled Mental Health Targeted Case Manager to join our team in Louisville. This rewarding role involves working closely with clients to help them achieve their recovery goals and live fulfilling lives.

 

Key Responsibilities:
  • ·
Care Coordination: Develop and implement personalized care plans that address the unique needs and challenges of each client, ensuring they receive comprehensive support throughout their recovery journey.
  • ·
Client Engagement: Foster strong relationships with clients, leveraging your exceptional communication skills to build trust and encourage open dialogue about their progress, struggles, and goals.
  • ·
Resource Linkage: Identify and connect clients with vital community resources, such as healthcare services, educational programs, and employment opportunities, to promote holistic well-being.
  • ·
Documentation: Maintain accurate and timely records of client interactions, progress, and outcomes, ensuring seamless communication among team members and stakeholders.
  • ·
Collaboration: Work collaboratively with internal and external teams to ensure clients receive coordinated and high-quality care, addressing complex needs and barriers to recovery.
  • ·
Professional Development: Participate in ongoing training and professional development opportunities to enhance your skills and knowledge, staying up-to-date on best practices in mental health and substance abuse treatment.

 

Requirements:
  • · Associate or Bachelor's degree in Social Work, Human Services, or related field.
  • · 1+ year(s) experience in an AOD or mental health setting.
  • · Active CDCA or LCDCII/ LCDCIII/ LICDC licensure from the Kentucky Chemical Dependency Professionals Board.

 

Preferred Qualifications:
  • · Strong interpersonal and engagement skills.
  • · Excellent written and verbal communication skills.
  • · Strong computer skills required; experience with documentation in EMR.

Salary Information:This fee-for-service position offers a competitive salary range of $50,000 - $65,000 annually, with 80% of work being billable hours and 20% administrative hours.
