The Client Care Coordinator plays a pivotal role in overseeing the client journey from initial referral through to ongoing care continuity.
This position commences in the community, where the Coordinator collaborates with the Operations Manager to cultivate and sustain partnerships with referral entities such as assisted living facilities, hospice organizations, geriatric care managers, medical professionals, and other senior service referral networks.
Subsequently, the Coordinator engages with potential clients by responding to referral inquiries, providing comprehensive information about our services, and arranging in-home consultations.
Upon the client's enrollment, the Coordinator is tasked with ensuring seamless care delivery by coordinating family resources and liaising with various stakeholders, including community service providers, insurance representatives, case managers, and other third-party payers.
Ultimately, the Coordinator will collaborate closely with the Internal Care Coordinator (ICC) to guarantee that caregiver assignments align with client needs, thereby ensuring an exceptional experience for our clients.
Job Requirements:
- High school diploma or GED is required.
- One to three years of relevant experience in a comparable role.
- Proven expertise in marketing and sales with a successful track record.
- Strong leadership capabilities and excellent interpersonal skills.
- Exceptional customer service and communication skills, both written and verbal.
- Willingness to spend significant time in the field, with driving as a necessary component.
- A dependable vehicle and valid driver's license, with the ability to pass a background check (including motor vehicle records).
Working Environment:
- Office setting.
- Responsibilities include walking, sitting, and standing, with lifting generally limited to files and records not exceeding 5 pounds.
- Extended periods spent at a computer workstation and on the phone.
- Significant time spent in a vehicle.
