Key Responsibilities:
  • Conduct regular rounds on assigned patients to assess their progress and adherence to the care plan.
  • Effectively communicate the care plan to patients and their families, addressing any concerns or questions that arise.
  • Provide emotional support, educational resources, and expert guidance to empower patients and families in their healthcare decisions.
  • Act as a liaison among patients, clinical staff, administration, and external resources to optimize care coordination and resolve any barriers.
  • Assist patients and families in navigating financial, psychosocial, and administrative challenges by offering options and referrals to appropriate resources.
  • Utilize critical thinking and decisive judgment to operate effectively with minimal supervision in a fast-paced environment.

Qualifications:
  • Licensure: Must possess a valid Registered Nurse (RN) license.
  • Education: Graduate of an accredited nursing program; a Bachelor's degree in Nursing is preferred.
  • Experience: A minimum of 2 years of nursing experience is required.
  • Certifications: Current BLS certification from an accredited organization is mandatory prior to the start date.
