Position Overview

The role encompasses the Alma and Mt Pleasant regions.


The primary responsibility is to adhere to the individualized care plan for each patient, providing assistance with personal care tasks when the patient or their family is unable to do so.

The aide will support patients with various diagnoses and at different stages of their health journey.

Key Responsibilities

(100%) Deliver hands-on personal care while demonstrating proficiency in assigned duties; follow the patient’s care plan to ensure completion of designated tasks. Execute basic procedures as an extension of nursing services. Aid in mobility or exercises. Adhere to infection control protocols. Foster positive relationships with patients and their families. Uphold patient rights. Collaborate as part of an interdisciplinary team. Report any changes in the patient’s condition to a registered nurse. Maintain accurate records in accordance with policies and procedures.

ADDITIONAL DUTIES:

Encourage patient independence and assist or perform personal care tasks (such as bathing, dressing, grooming, oral hygiene, skin care, toileting, changing bed linens, and other personal care activities).


Accurately measure and document vital signs including temperature, pulse, respiration, and blood pressure as required by the care plan, promptly reporting any abnormal findings to the appropriate personnel.

Ensure skin integrity by maintaining cleanliness and dryness, employing pressure prevention techniques as outlined in the care plan. Monitor for any changes in skin condition and report observations to the nurse or manager.


Utilize proper techniques for repositioning patients in bed, during ambulation, or while transferring to and from beds or chairs.

Maintain a safe and secure environment during the provision of care.


Adhere to the patient’s care plan and document care provided along with any relevant information concerning the patient and/or family.

Ensure timely documentation as defined by agency standards.

Consistently exhibit a positive, courteous, and helpful demeanor towards patients, families, and all stakeholders during every interaction.


Function effectively as a team member, demonstrated through positive verbal and non-verbal communication to support healthy relationships and achieve organizational objectives.

Maintain knowledge of and comply with organizational policies and procedures.

Stay updated on skills and knowledge regarding equipment usage.

MyMichigan Health is a technology-driven organization; employees are expected to demonstrate proficiency in Microsoft Windows.

Perform other assigned duties as necessary.

Certifications and Licensures

PCT:
Certification as a Patient Care Technician/CNA/EMT/LPN from MyMichigan Health


New hires post-January 2018 must possess an internal MyMichigan Health Patient Care Technician certification or a State of Michigan CNA certificate.

There must not be a lapse of more than 24 months in providing compensated services, or the individual must complete another CNA program to meet job qualifications.

Alternatively, candidates may complete a competency evaluation through written or oral examination or observation of the aide performing the competency.

For aides employed prior to January 2018: Documentation of completion of a home health/hospice aide competency evaluation through written or oral examination or observation is required.

To be completed within one month of hire or transfer.

Vehicle Insurance

FINGERPRIN:
Fingerprinting

TEAM ADVANCED:
Techniques for Effective Aggression Management

To be completed within six months of hire or transfer (Employees in this position on or before 12/31/2023 will have 24 months to complete this certification).

Required Education

A high school diploma or GED is mandatory for this position.

Additional Information

EXPERIENCE, TRAINING, AND SKILLS:


Attendance at required meetings/in-services is expected; aides must complete a minimum of 12 hours of continuing education/in-service training as defined by the organization within a 12-month period of employment.

Employees may be required to engage in further learning opportunities provided by MyMichigan Health.

Successful completion of the Home Health Missouri Alliance Aide test with a score of 85% or higher is required.

Access to reliable transportation is essential.

Ability to drive under varying road conditions.

Effective communication skills, both verbal and written, and proficiency in understanding, speaking, and writing in English are necessary.

Ability to connect with individuals of all ages and diverse cultural and spiritual backgrounds.

Capacity to work with patients and families facing terminal illness, the dying process, and death.

Availability to work weekends, be on-call as scheduled, and maintain flexibility in work hours.

PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS:

Exposure to stressful situations, including public contact, trauma, grief, and death.

Ability to wear personal protective equipment, including latex materials or suitable alternatives as required for the position.


Must be able to move freely throughout the facility with or without assistive devices and perform the functions of the job as outlined in the job description.

Overall vision and hearing capabilities are necessary, with or without assistive devices.

Frequent requirements to sit, stand, or walk for extended periods. May involve frequent postural changes such as stooping, kneeling, or crouching.

Some exposure to bloodborne pathogens and other potentially infectious materials. Compliance with MyMichigan Health bloodborne pathogen and TB testing protocols is required.


Ability to manage multiple tasks, collaborate effectively with others, work independently, maintain regular attendance, and stay alert is essential.

Overall dexterity is required, including handling, reaching, grasping, fingering, and feeling, with the potential for repetitive movements on a regular to frequent basis.

Physical Demand Level:
Medium. Must be capable of occasionally lifting or carrying 21-50 lbs. (0-33% of the workday), frequently lifting or carrying 20 lbs. (34-66% of the workday), and constantly lifting or carrying 10 lbs. (66-100% of the workday).
