Overview

This position is responsible for providing care in the Alma and Mt Pleasant areas.


The role involves adhering to the personalized care plan for each patient, assisting with personal care tasks when the patient or their family is unable to do so.

The aide will support patients across various diagnoses and stages of health.

Key Responsibilities

(100%) Deliver hands-on personal care while demonstrating proficiency in assigned duties; Follow the individualized care plan to ensure completion of tasks. Execute basic procedures as an extension of nursing services. Aid in mobility or exercises. Adhere to infection control protocols. Foster positive relationships with patients and their families. Uphold patient rights. Collaborate as part of an interdisciplinary team. Report any changes in the patient's condition to a registered nurse. Complete necessary documentation in accordance with established policies and procedures.

Additional Duties:

Encourage patient independence and assist or perform personal care tasks (such as bathing, dressing, grooming, oral hygiene, skin care, toileting, changing bed linens, and other personal care activities.)


Accurately measure and document vital signs, including temperature, pulse, respiration, and blood pressure as required by the care plan, and promptly report any abnormal findings to the appropriate personnel.

Maintain skin integrity by ensuring cleanliness and dryness, and utilize preventative positioning techniques as outlined in the care plan. Monitor for any changes in skin condition and report observations to the nurse or manager.


Employ proper techniques for repositioning patients in bed, during ambulation, or while transferring to and from beds or chairs.

Ensure a safe and secure environment while providing care.


Adhere to the patient's care plan and accurately document the care provided along with any relevant information concerning the patient and/or family.

Documentation must be completed in a timely manner as per agency standards.

Consistently exhibit a positive, courteous, and helpful demeanor towards patients, families, and all customers during every interaction.


Function effectively as a team member, demonstrating positive verbal and non-verbal communication to support healthy relationships and achieve organizational objectives.

Maintain familiarity with and adhere to organizational policies and procedures.

Stay updated on skills and knowledge regarding equipment usage.

Mid Michigan Health is a technology-oriented organization, and employees are expected to demonstrate proficiency in Microsoft Windows.

Perform other assigned duties as necessary.

Certifications and Licenses

DRIVERSLIC:
Valid Driver's License

PCT:
Certification as a Patient Care Technician, CNA, EMT, or LPN from MMH


New hires must possess either an internal MyMichigan Health Patient Care Technician certification or a State of Michigan CNA certificate.

There should not be a gap of more than 24 months in providing compensated services, or the individual must complete another CNA program to meet job qualifications.

Alternatively, completion of a competency evaluation through written, oral, or observational assessments may be required.

For aides employed prior to the specified date: Documentation of a completed home health/hospice aide competency evaluation through written, oral, or observational assessments is necessary.


BLS:
Basic Life Support certification

Must be obtained within one month of hire or transfer.

VEH INS:
Vehicle Insurance

FINGERPRIN:
Fingerprinting

TEAM ADVANCED:
Techniques for Effective Aggression Management

Must be completed within six months of hire or transfer (employees in this position prior to the specified date will have 24 months to complete this certification).

Required Education

A high school diploma or GED is required for this position.

Other Information

Experience, Training, and Skills:


Attendance at meetings and in-services is mandatory; aides must complete a minimum of 12 hours of continuing education/in-service training as defined by the organization within a 12-month period of employment.

Participation in additional learning opportunities offered by Mid Michigan Health may be required.

Successful completion of the Home Health Missouri Alliance Aide test with a score of 85% or higher is necessary.

Access to reliable transportation is essential.

Ability to drive in varying road conditions is required.

Effective communication skills, both verbal and written, along with proficiency in understanding and using the English language, are necessary.

Ability to connect with individuals of all ages and diverse cultural and spiritual backgrounds is important.

Capability to work with patients and families facing terminal illness, the dying process, and death is essential.

Availability to work weekends, be on-call as scheduled, and maintain flexibility in the work schedule is required.

Physical/Mental Requirements and Typical Working Conditions:

Exposure to stressful situations, including public contact, trauma, grief, and death.

Must be able to wear personal protective equipment, including latex materials or suitable alternatives as required for the position.


Ability to move freely throughout the facility with or without assistive devices and perform job functions as outlined in the job description.

Overall vision and hearing are necessary, with or without assistive devices.

Frequent sitting, standing, and walking for extended periods are required. May involve frequent postural changes such as stooping, kneeling, or crouching.

Some exposure to bloodborne pathogens and other potentially infectious materials. Compliance with Mid Michigan Health's bloodborne pathogen and TB testing requirements is mandatory.


Ability to manage multiple tasks, collaborate with others, work independently, maintain regular attendance, and remain alert is essential.

Overall dexterity is required, including handling, reaching, grasping, fingering, and feeling. May require repetition of these movements regularly.
Physical Demand Level:
Medium. Must be capable of occasionally lifting or carrying 21-50 lbs., frequently lifting 20 lbs., and constantly managing 10 lbs. throughout the workday.
