About the RoleAs a Resident Assessment Nurse (LPN), you will lead an interdisciplinary team in conducting comprehensive assessments of residents. You will work closely with the Director of Nursing and the Care Plan Team to develop and implement individualized care plans that meet the unique needs of each resident. Your attention to detail and ability to think critically will be essential in ensuring that residents receive high-quality care.
Key Responsibilities- Coordinate the interdisciplinary assessment process for assigned residents
- Develop and ensure completion of Comprehensive Care Plans in accordance with company, state, and federal guidelines
- Monitor residents to ensure that individual care plans are followed and updated as necessary
Our Resident Assessment Nurse (LPN) role offers a unique blend of administrative and clinical responsibilities. You will have the opportunity to build meaningful relationships with residents and their families, and to make a positive impact on their lives. Additionally, you will have the potential to pursue certification as a Resident Assessment Coordinator (RAC-CT) through AANAC.
Requirements- Current State license as a Nurse in good standing
- MDS experience preferred but not required; we will provide training
- Knowledge of general nursing standards of care and medical practices
- Excellent analytical and deductive reasoning skills
- Computer literacy and comprehensive understanding of electronic documentation programs
