About the Role

As a Resident Assessment Nurse (LPN), you will lead an interdisciplinary team in conducting comprehensive assessments of residents. You will work closely with the Director of Nursing and the Care Plan Team to develop and implement individualized care plans that meet the unique needs of each resident. Your attention to detail and ability to think critically will be essential in ensuring that residents receive high-quality care.

Key Responsibilities
  • Coordinate the interdisciplinary assessment process for assigned residents
  • Develop and ensure completion of Comprehensive Care Plans in accordance with company, state, and federal guidelines
  • Monitor residents to ensure that individual care plans are followed and updated as necessary
