Job Summary:

The RN Unit Care Coordinator is a vital member of our team at Villa Manor Care Center, responsible for overseeing and implementing patient care plans in accordance with Life Care standards. This role requires a strong nursing background, with a focus on geriatric care and a passion for delivering exceptional patient outcomes.

Key Responsibilities:

* Supervise and coordinate patient care activities on the unit, ensuring seamless delivery of services and high-quality patient outcomes.
* Collaborate with nursing staff, CNAs, and other healthcare professionals to develop and implement patient care plans that meet individual needs and goals.
* Provide leadership and guidance to unit nurses and CNAs, promoting a culture of excellence and teamwork.
* Maintain accurate and detailed records of patient care activities, including progress notes and treatment plans.

Requirements:

* Current RN licensure and a minimum of one year of geriatric nursing experience.
* Advanced knowledge of geriatric care principles and practices, with a focus on patient-centered care and population health management.
* Strong leadership and communication skills, with the ability to work effectively with diverse stakeholders, including patients, families, and healthcare professionals.
* Ability to maintain confidentiality and handle sensitive patient information with discretion.
