Job Summary

We are seeking a highly skilled Care Coordinator to join our team at Summit Medical Consultants LLC. As a Care Coordinator, you will play a vital role in providing medical and clinical support functions in the long term care/skilled nursing setting.

Key Responsibilities
  • Facilitate communication between healthcare providers, facility staff, and administrative teams.
  • Complete Chronic Care Management for LTC/SNF patients within assigned facilities.
  • Provide support to onsite providers, including collecting vital signs, relaying verbal orders, and assisting with medication orders.
  • Perform venipuncture and/or lab draws, IV insertion, and documentation of patient medical history.
  • Attend IDT/care conference meetings and review patient medical records.
  • Assist with discharge planning, paperwork, and managing incoming medical records/authorization requests.
Requirements
  • LPN active license.
  • Minimum of 1 year Case Management or Post acute care experience.
  • Experience in use of Electronic Medical Records.
  • Strong written and verbal communication skills.
  • Experience with Google suites and smart sheets preferred.

Work Environment

This is a full-time position with a competitive salary of $25-$32 per hour based on experience and licensure. The work schedule is Monday to Friday, with on-the-road work location.
