Key Responsibilities:
- Manage chronic disease care for patients through telephonic communication.
- Assist in patient evaluations by gathering necessary information via phone.
- Deliver Chronic Care Management (CCM) services to enhance patient care quality and minimize hospital readmissions.
- Facilitate transitional care from hospital settings to other environments as outlined in care plans, collaborating with hospital coordinators.
- Support patients and families in accessing entitlements and community services, including those offered by the New York Cancer Foundation.
- Guide patients and families through treatment plans, emphasizing self-care, symptom management, and available resources.
- Collaborate with in-office healthcare providers to address daily patient inquiries related to symptom control and medication management.
- Educate patients on appropriate communication with healthcare providers for urgent matters.
- Collect clinical data to prepare patients for consultations with healthcare providers.
- Conduct medication reconciliations and verify allergies to maintain accurate medical records.
- Document all patient interactions clearly and in a timely manner, adhering to established protocols.
- Engage with healthcare professionals regarding patient conditions and care strategies.
- Encourage patients to adhere to follow-up appointments and self-care recommendations based on nursing assessments.
- Achieve daily and weekly objectives set by the Care Coordination Supervisor.
- Participate in ongoing professional development and training as required.
- Graduation from an accredited Nursing School.
- Valid New York State RN license.
- Experience with Electronic Health Records (EHR).
- Proficient in utilizing computer technology, including typing skills.
- Strong customer service, leadership, communication, interpersonal, and problem-solving abilities.
- Spanish language skills are preferred.
Compensation: Up to $48 per hour, commensurate with experience and education. Comprehensive benefits package includes Tuition Reimbursement, Health Insurance from day one, Dental, Vision, Life Insurance, Short and Long-term disability, 401k Plan, generous PTO, and 8 paid holidays (2 floating).
