Triage Registered Nurse

At New York Cancer & Blood Specialists, we're dedicated to providing exceptional patient care and fostering a collaborative environment for our healthcare professionals.

Key Responsibilities:
  • Provide chronic disease management for patients through telephone communication
  • Contribute to patient assessment by collecting data via phone
  • Conduct CCM services to improve quality patient care and reduce hospital readmissions
  • Coordinate transitional care from inpatient services to other settings
  • Assist with advocacy for patients and families regarding entitlements and community services
  • Develop and implement disease treatment plans, including self-care and symptom management
  • Work closely with in-office providers to manage day-to-day calls and provide patient education
  • Educate patients on proper communication with the physician's office and emergency services
  • Perform medication reconciliations, allergy verification, and review outside providers
  • Document patient communication and gather data according to established guidelines
  • Communicate with healthcare providers regarding patient status
  • Direct patients to follow-up appointments and maintain self-home care
  • Meet and report daily and weekly goals set by the Care Coordination Supervisor
Requirements:
  • Graduate from an accredited School of Nursing
  • Current NY State license to practice as an RN
  • Experience working with Electronic Health Record (EHR)
  • Proficiency with computer technology, including typing skills
  • Customer service, leadership, communication, interpersonal, and problem-solving skills
  • Spanish preferred
