At New York Cancer & Blood Specialists, we're dedicated to providing exceptional patient care and fostering a collaborative environment for our healthcare professionals.
Key Responsibilities:- Provide chronic disease management for patients through telephone communication
- Contribute to patient assessment by collecting data via phone
- Conduct CCM services to improve quality patient care and reduce hospital readmissions
- Coordinate transitional care from inpatient services to other settings
- Assist with advocacy for patients and families regarding entitlements and community services
- Develop and implement disease treatment plans, including self-care and symptom management
- Work closely with in-office providers to manage day-to-day calls and provide patient education
- Educate patients on proper communication with the physician's office and emergency services
- Perform medication reconciliations, allergy verification, and review outside providers
- Document patient communication and gather data according to established guidelines
- Communicate with healthcare providers regarding patient status
- Direct patients to follow-up appointments and maintain self-home care
- Meet and report daily and weekly goals set by the Care Coordination Supervisor
- Graduate from an accredited School of Nursing
- Current NY State license to practice as an RN
- Experience working with Electronic Health Record (EHR)
- Proficiency with computer technology, including typing skills
- Customer service, leadership, communication, interpersonal, and problem-solving skills
- Spanish preferred
- Salary up to $48 an hour based on experience and education
