Nurse Care Manager

We are seeking a skilled Registered Nurse to join our Transitional Care Unit team at Providence Mount St. Vincent in Seattle, Washington.
As a registered nurse on our team, you will work alongside an interdisciplinary group to provide whole-person care to patients in need. Our nurses are empowered by our shared governance model that includes nursing staff in the process of creating practice standards and expectations.
About the role:
You will be working in an intermediate and high-acuity department that facilitates recovery from serious injury, illness, disease or medical condition. From the moment patients leave the hospital, we guide and care for their physical, emotional and spiritual needs before they're able to return home, preparing them for the road ahead. Our Transitional Care Unit is an interdisciplinary haven for healthcare professionals interested in handling medical complexities in a more comfortable and home-like setting for the patients.

Required Qualifications:
- Education sufficient for licensure
- Upon hire: Washington Registered Nurse License
- Within 30 days of hire: National Provider BLS - American Heart Association
Preferred Qualifications:
- Bachelor's Degree Nursing
- Food Handler's Permit
- 1 year Med-Surg experience (Acute vs Sub-Acute)
- Supervisory experience with nursing assistants/CNAs
