Position Overview: Liberty Healthcare Corporation is seeking a dedicated full-time Investigator / Case Worker to join our Adult Protective Services Program in collaboration with the Pennsylvania Department of Human Services.

Program Mission: Our mission is to thoroughly investigate reports of potential abuse, neglect, exploitation, or abandonment affecting adults with disabilities, ensuring their safety and wellbeing.

Key Responsibilities:

  • Conducting thorough investigations in various residential settings.
  • Assessing the validity of reports and documenting findings meticulously.
  • Developing comprehensive plans to address identified risks.
  • Managing active cases with a focus on client welfare.
  • Traveling across multiple counties for case assessments.

Work Environment: This role offers a significant degree of autonomy and flexibility, including the possibility of evening hours. The position involves local and regional travel, requiring a personal vehicle for transportation.

Compensation and Benefits: The starting salary is approximately $47,000 annually, equating to $22.60 per hour. Additional income opportunities may arise through optional overtime and performance bonuses ranging from $750 to $1,500, awarded biannually. Employees are entitled to 30 days of paid time off each year, which can be utilized for various personal needs.

Qualifications: A Bachelor's degree is preferred; however, relevant experience in the following areas will be considered:

  • Behavioral health, mental health, or services for individuals with intellectual and developmental disabilities.
  • Field-based healthcare or human services.
  • Roles such as Service Coordinator or Program Specialist.
  • Experience in case management within Child, Family, and Youth Services.
  • Law enforcement or protective services experience.

Additional Skills: Familiarity with Social Assistance Management Systems (SAMS) and Home and Community Service Information Systems (HCSIS) is advantageous. Proficiency in Spanish is also beneficial.
