Position Overview: A full-time role is available as an Investigator / Case Manager within Liberty Healthcare, collaborating with the Pennsylvania Department of Human Services in the Adult Protective Services Program.
Mission: This program is dedicated to investigating reports of need and, when necessary, addressing or eliminating immediate risks of abuse, neglect, exploitation, or abandonment for adults with disabilities.
Key Responsibilities: As an Investigator / Case Manager, your primary focus will be on safeguarding the wellbeing of vulnerable individuals. Your daily tasks will include:
- Traveling throughout the designated region to conduct thorough investigations in various residences.
- Assessing the validity of reports and documenting your findings meticulously.
- Creating actionable plans and managing ongoing cases effectively.
Travel Requirements: This role involves travel across multiple counties, including Philadelphia, Chester, Bucks, Delaware, and Montgomery.
Work Environment: The position offers a challenging yet rewarding experience with a significant degree of autonomy. A flexible work schedule is provided, which may include occasional evening hours.
Compensation: The starting salary is approximately $47,000 annually, equating to $22.60 per hour. There are opportunities for additional income through optional overtime and extra case assignments. Eligible employees can also receive biannual performance bonuses ranging from $750 to $1,500.
Benefits: Employees will accrue 30 days of paid time off each year, which can be utilized for holidays, vacations, personal time, and sick leave. Additional benefits are also available. Comprehensive training in adult protective services will be provided to new hires.
Qualifications: A Bachelor's degree is preferred; however, relevant experience may substitute for formal education. Acceptable experience includes:
- Behavioral health, mental health, or services for individuals with intellectual and developmental disabilities.
- Field-based healthcare or human services experience.
- Roles such as Service Coordinator, Supports Coordinator, or Program Specialist.
- Experience in child, family, and youth services case management.
- Law enforcement background.
- Experience in case management or protective services.
Additional Requirements: Candidates must possess their own vehicle for local and regional travel, with work-related mileage and toll expenses reimbursed. Familiarity with Social Assistance Management Systems (SAMS) and Home and Community Service Information Systems (HCSIS) is advantageous, and proficiency in Spanish is a plus.
