Job Title: Home Health RN Case Manager

VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health RN Case Manager. This is a full-time position that requires a strong commitment to delivering high-quality patient care in a home health setting.

Job Summary:

The Home Health RN Case Manager will be responsible for providing comprehensive nursing care to patients in their homes, working closely with healthcare providers and other stakeholders to ensure seamless care coordination. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Provide direct and indirect nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with healthcare providers, family members, and other stakeholders to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop plans to address these needs.
  • Monitor patients' progress, identify potential issues, and make adjustments to care plans as needed.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Document patient information, including assessments, care plans, and progress notes, in a timely and accurate manner.
Requirements:
  • Current RN licensure in the State of Ohio.
  • 1-2 years of experience in home health care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and CPR certification.
