Home Health RN Manager Job Description

VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to fill a Home Health Manager position in Warrensville Heights, Ohio.

Job Summary:

The successful candidate will be responsible for providing comprehensive nursing care to patients in their homes, working closely with healthcare teams to ensure seamless care coordination.

Responsibilities:

  • Utilize the nursing process to assess patient needs and develop individualized care plans.
  • Collaborate with healthcare providers to ensure effective care coordination and communication.
  • Conduct thorough assessments and evaluations to identify patient needs and develop strategies for improvement.
  • Develop and implement education programs for patients and families to promote health and wellness.
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.

Requirements:

  • 1-2 years of experience in home health care or a related field.
  • Completion of an accredited RN education program with a diploma, Associate's or Bachelor's degree.
  • Valid driver's license and CPR certification required.
  • Bi-lingual a plus.
